Discrimination Complaint Report Form

The University seeks to ensure a positive living, learning, and working environment for all LMU community members. Specifically, this Policy prohibits unwelcome, harassing or discriminatory conduct on the basis of race, religious creed, color, national origin, ancestry, physical disability, mental disability, legally protected medical condition (cancer-related), marital status, sex (including pregnancy, childbirth, or related medical condition), age over 40, veteran status, sexual orientation, or any other bases protected by federal, state, or local law. Students, faculty, and staff should be aware of and avoid actions that others may construe as unwelcome and/or harassing.

The objective of the Policy is to prevent an environment from developing which unreasonably interferes with a student's academic endeavors and/or faculty and staffs' work or has the effect of creating an intimidating, hostile, or offensive environment because of such prohibited conduct. It is essential that students, faculty, and staff immediately report conduct that is believed to be in violation of this policy. Prompt reporting is necessary in order to conduct timely fact-finding about complaints, attempt to remedy the problem before the situation escalates, and reduce the potential for the spread of harmful rumors. In no event shall a complaint be filed later than one year from the date of the alleged conduct.

This form consists of two pages: (1) the first section below requests your contact information and (2) the following page provides room to describe the allegation in further detail (responses are limited to 100 lines of text).
